The Clinical Liaison RN (Home Infusion) supports home infusion pharmacy services by fostering referral relationships and coordinating patient transitions for infusion therapy. This role involves collaboration with healthcare providers to assess patient needs, provide education, and ensure documentation compliance. The position requires a licensed RN with clinical experience and involves consultative, educational, and liaison duties to enhance patient care and referral growth.

This position will be supporting Oregon Home Infusion Pharmacy.
The Home Infusion Clinical Liaison position is responsible for creating and maintaining positive referral relationships in order to increase Home Infusion Referrals within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and other individuals to assess appropriateness home infusion, facilitate patient care transitions onto service and ensure there is appropriate documentation for ordered infusion therapy. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding home infusion therapy, and serving as a clinical resource and educator for ordering providers, facility staff, patients, families on Home Infusion Therapy.
